Section-Excerpt-Chapter generates a blank page between Excerpt-Opener and Excerpt-Chapter... #47

...in the pdf generated from this manuscript. Also the first page of the Excerpt-Chapter does not have a header in the pdf.

(Both of these items are different from the pdf generated using xsl html conversion.)
