For an epub with only one Chapter, set its default ncx entry to 'Begin Reading' #48

In this test manuscript, when we restyled and removed the ChapTitleNonPrinting, the auto-inserted h1 is just called 'Chapter', and that's what ends up in the ncx. We may want to set that to 'Begin Reading' for a book with only one Chapter... or is that more trouble than it's worth / a non-optimal outcome in cases with Sections like Intro-Chapters & Prefaces?
