OPL3 emulator - restart of ADSR envelope is buggy under some circumstances #22

It looks like ADSR envelope is not restarted correctly under some circumstances 
(e.g. attack rate = 0). Very significant sound example is 
MODULES\MLF\TRACED_D.AMD. Altering instrument #2 -> #3 in first channel seems 
to be ok, but altering #3 -> #2 doesn't work like with the real OPL3 chip, as 
instrument #3 has modulator's attack rate = 0.
