We use a non-standard extension for Makefiles. I want the make language syntax highlighting, but I can't tell VS that my extension is a Makefile because this extension doesn't register all the supported languages with VS in a way to let me pick it from the dialog in Tools / Options / Text Editor / File Extension. I guess that's picking an "Editor" so maybe you only need to register TextMate as an "editor"?
