Integrated band member, instrument, connection, and monitor requests #97

As much as I hate compound bugs, this is a feature request that's going to cut across a lot of different things. If this needs to be broken down into separate bugs with dependencies (unsure if GitHub can do that?), please indicate. This includes elements of Issue 90, Issue 58, and Issue 8.

The primary (input) feature that the other features depend on is:

As a band, I want all information about my band, its members, and its instruments to be requested in one place, and in a well-guided fashion.

The dependent (reporting) features are:

  1. As somebody responsible for AV RFPs, I should be able to determine my backline requirements based on submitted band information.
  2. As somebody responsible for AV RFPs, I should be able to determine my mic/stand/DI requirements based on submitted band information.
  3. As somebody responsible for stage/soundboard integration, I should be able to get a first cut of a channel plan (including which channels need phantom power) based on submitted band information.
  4. As somebody responsible for stage/soundboard integration, I should have advance knowledge of the desired monitor mixes to streamline monitor setup during changeover.
  5. As somebody responsible for band changeovers, I should be able to determine what equipment will need to be brought on/off stage between acts

Please find attached a draft form template. Note that it may not cover the totality of band information -- such as the point-of-contact -- that might be worthwhile to integrate. However, it should collect the information needed to cover the reports I've outlined above.

An example (fictitious) band, The Fictitious. The band name is prominently absent in this questionnaire... I do assume that's already asked elsewhere in the system. :)
