Additional Info Placeholder-Name field not implemented fine #31

WordPress version: 5.6.1 Area Path: Admin Panel --> Get A Quote --> Configuration --> Form Fields --> Edit Status: new Version: 1.0.0 Description: The Additional Info Placeholder-Name field is getting visible and hidden based on the previous selected field for editing and changing the place holder text as well accordingly.

Placeholder-Name field is removed, working fine for now.
