Add enrollment for course for not-connected providers #368

As a user, I want to mark (and unmark) courses as enrolled even if the MOOC provider does not support enrollments with an API.

In addition, there should be a possibility to remove enrollments (API-added enrollments and individual ones). Refer to #439, which is no bug.
