USGS web services were down for some period of time and as a result this tool wasn't operational, resulting in timeout error. USGS web services are now operational so this tool should work as expected.
I just tested the parameters in the screenshot with the same address and service returns data.

For the recent few days, I have received an error message when attempting to look up seismic parameters (using both address and coordinate):
Let me know if there is a workaround. This is present on all USGS powered hazard maps sites (ATC Hazards by Location, ASCE 7 Hazard Tool, etc.).
