Target Mapnik API versions #433

Add support for targeting mapnik api versions from the command line and support minimum/maximum-scale-denominiator. Fixes #394.

Updated default Mapnik version to 2.3.0 so that no existing setups will break after this is merged. I think Mapnik 3 will not be widespread enough that a default of 3.0.0 is warranted. This should change in the future though.
