Create offboarding procedure

Per #182... A procedure is needed to offboard chapters when they are disbanding. Choice of options below depend on if we think there might be potential to reform chapters in the future and how much effort might be involved to archive vs delete online presence.

Potential steps:

1) Confirm that all current organizers wish to step down and inquire if an attempt was made to recruit replacements.
2) Inquire about the reasons for disbanding and ask if there is anything that HQ can do to help retain chapter. Perhaps create some kind of short survey.

If proceeding with offboarding...
3) Ensure that we have valid contact info for organizers in case future contact is required.

4) Twitter options:

• Asking chapter to close twitter account.
• Asking chapter to turn over account to HQ, who might consider a final tweet and changing the profile to reflect that chapter is defunct and is in need of interested organizers.

5) Website options:

• Deleting chapter repo or edit code to prevent hosting at maptime.io. Remove from chapters.json
• Edit chapter site and chapters.json to reflect that chapter is defunct. This might require some code changes to maptime.github.io including the map so that people are clear that there is not active chapter.

6) Remove organizers from github org and from slack
