Removing markups does not remove notes and comment tags #145

Create a markup and give it a "notes" and/or "summary". Then delete the
markup. You will see that the notes and/or summary elements are left
floating around as siblings of the segment/content marked up.

This screws up the logic behind text selection which does not expect notes
and summary elements to be siblings of segment/content etc.
