XQuery: Make Categories page better #164

This should show all categories (grouped under concepts) - and each group
sorted by markupType usage. (same logic as the search)

A to Z links at the top (just like the concepts page)

Categories hyperlinked to markup instances.

Note: all information should be in reference file (no need to search over all
transcripts). No data regarding markup types should be used
