Write Stan files to generate simulated data #13

This includes generic simulated data as well as the data that generates the pathological response data. This makes reproducibility platform independent.

As discussed in our meeting on 30 Oct, deciding how to simulate data is equivalent to deciding what prior to use for each pathology. For each of the issues that deal with specifying a prior, a Stan file that generates data should also be specified. This issue is now absorbed into the issues that talk about specifying a prior.
