Add clever randomization for a third pathology, respondent quality.

@RogerOverNOut the respondent-quality branch has all of Jeff's changes.

Changes merged into joint-pathologies branch. Given issues with implementing screening, trying now with a joint pathology that consists of ANA and respondent quality.
