Add description field to new poll page #96

Break out of #93 need to create a description field to the new poll creation page.

i guess we are doing this done c20cca93fcf0579f9c0bbf21dcd292f29617d239 originally had textarea, but voting page didn't show newlines, so changed it to single line input

Description field is working, possible adjustments should be split off into their own issue.
