XML Ingest changes

This ticket brings together a set of changes to XMLIngest aimed at making the 
component an ICAT plugin whilst improving code the quality and generality. As 
this is only of interest to ISIS I am rolling it into a single ticket.

It is a merge of issues #8 , #12 , #13 , #22 , #33 , #39 , #40 , #57

The required changes are:

- Move XMLIngest to be an optional plugin rather than part of the main API
- Remove ISIS specific rules (make them parameters/properties) and document 
these
- Implement a queue system (MDBs?)
- Ingest investigation start date
- Check and confirm that it is using the latest XSD and is doing so properly
- Fix / remove 'tolower' calls

It would be good to have an overhaul of the XML handling to deal with data 
ingest, export and migration which is also able to deal with the setting up of 
an ICAT
