Doco: theme integration #156

Add diagrams to the theme integration posts.

Make a screenshot of the default theme with our form.
Draw outlines around different areas of the screen and insert references to
the functions that produce them.

There will be more than one screenshot, I suspect. We have single page,
multipage, etc.

This is purely for illustration purposes to make understanding of the doco
simpler.
