Too long to load previews #202

A template has 2 pages.
1. for some reason no default preview is shown
Another issue was posted re, this.

The 2 pages thumbs take too long to load. Why?
If it is possible for them load with a large delay then we either need to
show placeholders or loading indicators.

A user won't be happy looking at a half-blank screen for too long.

We need to add a loading indicator and some default image placeholders to tell 
the
user there is something loading. 
Preview and thumb sizes are known in advance, so we can show real size 
placeholders.
If not, show some default size and a loading indicator, same as elsewhere 
through the
site.

1. Loader shows up until the page preview is loaded (loader.png). Don't see a 
loader on page thumbs but they load pretty fast for me - Test OK

2. Thumbs turn blank after I generate a preview for a page (blank_thumb_1.png).

I wasn't able to see a loader in the thumbs, but they do load before all. 
Loader shows up while loading preview image.
