Add two-level TOC #381

It makes e-book much easier to navigate.

Thanks, but this would be a bit fragile to keep up to date. I've pushed d029eb7ac51df8b8 instead, so that I don't have to manually update the TOC every time I change a section title.

Absolutely! Headings are not carved in stone and this should be step in building process. Thank you.
