fix some BlockDiagIEB AD issues #87

@Karthikprabhu22 when you get a chance, could you verify this doesn't break your T+P MUSE w/ implicit-diff runs? (pkg> add CMBLensing#BlockDiagIEB_AD to get on this branch). I will merge after.
