I see 2 entries for every punch-in when i click "edit" for an activity

What steps will reproduce the problem?
1. Punch-in an activity and enter a comment and click save notes.
2. After 5 minutes, punch-out
3. Click "Edit" on the activity to open a new window.
4. You will notice that there are 2 entries. One without a comment and one 
with a comment

What is the expected output? What do you see instead?
Just one entry with a comment.

What version of the product are you using? On what operating system?
Activity Tracker Plus on iGoogle

Please provide any additional information below.

Hi,
  The 2 entries you see are for each change you did:
1. For punch-in
2. For adding a comment.

   The 2 entries are available so that if you want to remove the comment, all you do
is delete the entry with the comment and you don't have to delete the other 
entry.
