QA: Upgrade to Postgres 16 #230

The latest version of Postgres is Postgres 16. Upgrade the QA environment to use Postgres 16 by default.

The QA environment and docker configuration has been updated to use Postgres 16 by default. Postgres 15 can now be run standalone on demand.
