Quick update to address alignment issues. See the screenshot in issue #89 for the issue. Now the checkbox should be aligned correctly. The checkbox also had a strange outline that is now gone as well. Finally, fixed the vertical alignment of the field names (they were at the top as seen in the screen shot in issue #89)
