EP map data clean up #33

@marks Need to clean up the outliers on the map at the states level. Many states have just a few outliers.

https://github.com/marks/ONC-EHR-Incentives-Program-Payout-Data/blob/master/public/data/ProvidersPaidByEHRProgram_Sep2013_EP/MANUAL_FIXES.rb was created to address these issues. Many of the outliers are gone though some issues remain.
