Custom start line numbers #21

Hi, when I was implementing luminous on my blog I wanted to have the possibility to set custom start line numbers, so I modified 3 files a little bit. I thought maybe more people would appreciate it :).

I've merged your changes in and added a few small tests and stuff for the new option (boring things).
