"Name" given to a problem should be unique and non-empty #26

Otherwise generates exceptions in the Student Progress Page.

@IvonArroyo Since Name is given by the author, they can write a duplicate name unless we append it with some unique hash. Why is there an exception because of "Name"? Aren't we using the problem ID on that page? Can you share the exception?

There are certain downstream components (Teacher tools, if I remember correctly) that expect the problem name to also be unique. It would be nice to enforce this at the time of question authoring.
