migrate "master" branch UI to "ionic" boilerplate #5

Initial UI has been created in "master" branch. We need to migrate those in Ionic branch. Screenshots are under screens folder.

One simple way to break down tasks:

add routes to point to page components
each page component can be a task (create placeholder event handlers, e.g., button click, on each page)
change side menu to map the menu created in master branch

Use ionic's components and default theme -- the are basically material style.

If anyone started working on filling the placeholder pages, please note that in data modeling doc, games and reciting are abstracted as a more generic concept reading activities. UI for these two should share an underlying form component, with the only different being type of the reading activity.
