Changelog for v4.0? #435

Having read the actual commit history, it appears I can just drop-in 0.4.0 on top of 0.3.12, but this isn't obvious or clear when 0.4.0 isn't in the changelog and the releases page has no details.

I've added a new changelog, and will be doing a better job keeping it up to date. Thanks!
