correctly handle KEY_B press in menus
optimize key combo handling
remove redundant workaround flag
remove redundant KEY_B checks
remove redundant ButtonMapper class & object

Thanks for this. Hopefully it solves some issues with button presses some people have. At least I have tested one combo that didn't work for me and with your PR it works now.
