When using the telemed-intake application, I have noticed that if a user does not complete the Questionnaire and they navigate back to the Patient form and proceed again they are asked to enter the Patients birthdate. Once they do this I see that the following endpoint is called zambda/create-appointment/execute which appears to create a duplicate Patient.
