Add an eRx module to Ottehr

Discussed with @rzinger. The code changes required for this are relatively small, but there are some problems with the current implementation that would introduce sign-in bugs to ottehr. Going to hold off on this until those issues are resolved.

working on pulling out medallion and adding a hook of sorts for external sync
