Tested on:

Ottehr Admin Version: 0.10.0 - Demo
Chrome Version 128.0.6613.137 (Official Build) (64-bit)

Steps To Reproduce:

Book a prebook visit for new patient
Proceed to paperwork
Select "Self" on the "Responsible party"

Expected behavior Birth sex is prefilled and locked DOB is prefilled and locked

Actual behavior Birth sex isn't prefilled and patient can enter value DOB isn't prefilled and locked
