Forms created in LibreOffice: Checkboxes and Radio Buttons not responsive

Expected behaviour

When filling out a PDF form exported from LibreOffice, users can toggle the checkboxes and radio buttons.

Actual behaviour

Users cannot toggle the checkboxes and radio buttons.

Steps to reproduce the behaviour

1. Open the attached LibreOffice file.
2. File > Export As > Export as PDF.
3. Use the settings in the attached image.
4. Click Export.
5. View the exported form in Atril. Checkboxes and radio buttons are not visibly responsive.
6. View the exported form in Firefox or Acroabat Reader. Checkboxes and radio buttons are visibly responsive.

After toggling the checkbox/radio button in Atril and attempting to close the PDF file, the prompt appears to save or close without saving. This indicates that internally the checkbox/radio button was altered internally.

MATE general version

1.26.0-1 (I think; appears doing dpkg -l | grep -i mate.)
