Yes, it is technically possible to disguise a RAT as an image by exploiting vulnerabilities in image processing software, such as CVE-2017-12149 in ImageMagick or CVE-2020-0601 in Windows. HOWEVER, this process is extremely complex and challenging to execute. Zero Day exploits
