0010 - Housekeeping commit: update MADR template and use as PR template #10

I am proposing a few minor changes to the MADR template, including adding a "Implementation Plan" section with checkboxes for each member code to encourage us to be specific about when changes are warranted in response to a decision.

I'm also setting template.md as the PR template for the repo.
