CAPTCHA Does Not Show on User Registration

What steps will reproduce the problem?
1. User Registration

What is the expected output? What do you see instead?
At the bottom of the User Registration there should be a CAPTCHA image, this 
image does not show.  

What version of the product are you using? On what operating system?
v 2.0.2

Please provide any additional information below.
The installation went well.  Everything works great except for user 
registration.  No CAPTCHA image appears.
