Create a CHANGELOG to track changes to hmu-rails

As a developer I need to know what has changed between different versions of hmu-rails so I know what to expect

Acceptance criteria

• [x] It should have a CHANGELOG.md file in the root of hmu-rails
• [x] It should follow the changelog conventions outlined on keepachangelog.com
