Donations...prompt for email #81

I don't have support for asking for someone's email when donating. Add that field.

FYI: Paypal has email if I want to use that, but it won't work for pledges to mail in checks.

Also, if I prompt for email, I can send an email with a thank you/reminder/instructions for mailing in a check.
