@mattdesl this is probably a less disruptive alternative to #133

This is a complex issue and I don't like things "automagically" appearing in the index.html based on unrelated flags, so I think I'll close this for now.

Users can use --base option if they want with pushstate, or just use absolute paths everywhere.
