Summary

Fixed the issue of page not found on clicking on disconnect button present on user connected successfully page.

Fixes https://github.com/mattermost/mattermost-plugin-jira/issues/1070

What to test

Run the /jira connect command
Complete with the login, you will be redirected to welcome page with close & disconnect button
Click on disconnect button

Exisitng

error 404 page not found error

Fixed

Successfully disconnected and redirected to mattermost with bot message regarding user disconnected
