Add new gross datasets #1

Since we started the project, LIS has released new gross datasets including Germany 2013 (de13), Norway 2013 (no13), Finland 2013 (fi13), Luxembourg 2013 (lu13), and United Kingdom 2013 (uk13). To include these surveys we need to:

  1. Capture social security contribution rates for imputations
  2. Update .do files
