Option to not require Entry Forms #350

Our competition finds that having entrants submit the full page entry forms 
with their entries wastes a lot of paper, and we never even look at the entry 
forms.  It would be useful to have a way to prevent entrants from printing and 
sending us those forms.  They would just affix their entry labels, and we could 
access the entry info from a computer if necessary.
