Allow to browse a list of HTML sub-chapters #108

What steps will reproduce the problem?
1. Enable sub-chaptering and open HTML file with <h*> headers

What is the expected output? What do you see instead?
Would like to to see a list of sub-chapters with their numbers and titles
and be able to jump to any of them. Right now Books allows only to navigate
through HTML sub-chapters sequentially.

What version of the product are you using? On what operating system?
Latest night build. 1.1.3.

Please provide any additional information below.
- Extremely useful for large single-file texts.
- Requested functionality will require (partial) caching of titles of
sub-chapters.

Sub-chaptering support is essentially removed from 2.x
