(Hopefully) fixes tntp/surveymonkey#27 and tntp/surveymonkey#62

thanks! I agree this should close #62, though I can't verify it, and it sounds like it solves your problem in #27? Then we can close that too, I just noted in that issue that my problem cases are obsolete.
