Add notebook and supporting code to analyze patterns of missing data, across multiple reasons for missingness.

What do you think about checking fig/ into the repo? I don't see it in gitignore. Otherwise LGTM.

we should add fig. I'm not sure mine is up to date now, but I'll sort it out and make another PR later.
