fix: placeholder missing titles for issue templates

This pull request includes updates to the issue templates to provide default titles for new issues, making it easier for users to understand what information is needed.

Updates to issue templates:

• .github/ISSUE_TEMPLATE/1-bug-report.yml: Changed the title field to provide a default value of "... is broken" to guide users in reporting bugs.
• .github/ISSUE_TEMPLATE/3-help-wanted.yml: Changed the title field to provide a default value of "I need help with..." to guide users in requesting help.
