Fix ../../ filenames #134

What steps will reproduce the problem?
1. trucov report
2. trucov dot_report
3. trucov graph_report

What is the expected output? What do you see instead?
 ../../foo.cpp
should be
 _..##..#foo.cpp

Functional tests check this for report, dot_report, and graph_report.
Verify this works, and verify those cases have been added to functional tests 
and pass.
