You're right, Telephone should be able to play ringtones right away. I haven't
tested this with .aiff files, though.
Is it the same when you're using .mp3 files?

MP3 files played right away. Only the .aiff files had any trouble.

FYI, I added two new ringtones tonight. One was a midi file that didn't work
as-is so
I converted it to MP3 using an online converter. The mp3 played right away. The
other
was an iTunes m4r ringtone file. I used SoundStudio to convert it to mp3, but
that
mp3 wouldn't play until I restarted Telephone. This is using 0.14.3 on Leopard
10.5.8

Additionally, it would be helpful if the supported formats were listed
somewhere such
as the FAQ or maybe the preferences menu. I came across mp3 by trial-and-error
and
.aiff only after reading this post.

I must report I just experienced the same problem under OS 10.6.3 with
Telephone 0.15.0 (25) with both MP3s and .aiffs. I had to restart Telephone to
hear the ring.
