Fork a version for Buergeramt #28

The current implementation is meant for medical institutions. The staff manages the queue.

Create a fork for the buergeramt which is different because:

users can queue themselves
- no need for patient management (patman.js)
staff only moves and calls users
we need a mechanism which verifies that users are legit and actually at the institution

Did I miss anything? Lets fork off and collect all todos.

Fork off or just tag the current version as "medical" and rewrite from here...
