Add on-screen indicators and error messages for required field data

Problem

There are no on-screen indicators or notifications of required or missing data in fields.

It also appears that the first and last name fields are not currently required but should be. See screenshot.

Solution

Add visual clues, such as an asterisk, and "error" highlights that call attention to fields that are missing required data when attempting to proceed.
